What Is GEO for Med Spas? Generative Engine Optimization Explained

GEO, generative engine optimization, is the work of putting your med spa’s facts on the page as plain text so AI search tools like ChatGPT and Google AI Overviews can read them and use them in an answer. We scanned 187 med spas across 10 US metros; 177 had a scorable Botox or primary-injectables page, and we evaluated only its first 100 words. 61% stated two or fewer of the five facts a patient decides on, and the two that vanish most are the ones that matter most: a price appeared in only 11% of those openings and a named, credentialed provider in 7%. Yet when we asked Google “how much does [treatment] cost,” an AI answer came back every time, 50 of 50 queries, built overwhelmingly from clinic websites. The mechanism is simple: AI answers the questions patients ask, and publishing concrete facts makes those facts available to retrieve and quote; withholding them does not. This is our own data, and below is the method, checks, and limits, because a number is only worth citing if you can see how it was made.

What does GEO mean, in plain terms?

When a patient asks ChatGPT a question or pulls up a Google AI Overview, the system reads web pages, extracts facts, and writes an answer. If your page states the right facts as plain text, it has a chance to contribute to that answer or be cited by name. If those facts are missing, buried in a booking widget, or locked inside an image, the system has nothing to work with. A page can look polished to a human visitor and still be nearly blank to an AI reader — the two experiences are separate, and GEO is what closes the distance between them.

How is GEO different from SEO?

SEO gets your med spa’s page listed in search results. GEO gets your med spa named or cited inside the AI answer a patient reads.

SEO and GEO overlap more than they compete, but their goals differ enough that running only one leaves the other underserved.

Does a med spa need SEO or GEO? Both, and for most clinics they are the same underlying work. SEO is the commercial side of getting found, and our med spa SEO guide covers it; GEO decides whether the AI answer names the clinic.

Schema markup can support both, but it is not a shortcut for either. We have seen pages with no schema at all appear in AI answers, and schema-heavy pages that never surface. The variable that matters is whether the plain-text facts a patient would ask for are present and findable.

Google does not treat GEO as a separate ranking system. Its 2026 AI-optimization guidance says the generative AI features run on the same core Search ranking and quality systems, and that optimizing for generative AI is still search optimization. We use GEO as a cross-platform term for a different outcome: a clinic’s facts being retrieved, cited, or used inside an AI answer — on Google and on systems like ChatGPT and Perplexity.

Where does AI get information about med spas?

Across our tests and scans, four sources seem to contribute.

Your service pages

The most direct layer. When ChatGPT or Google’s AI features pull information about a clinic, they reach for the clinic’s own pages — the price, the provider and credentials, the procedures, and the city.

Your Google Business Profile

Google’s local-ranking docs say a complete profile lifts local visibility, and its 2026 AI-optimization guidance now says directly that a complete Business Profile helps supply the local-business information that can appear in AI answers. It does double duty.

Your review profile

A current, genuine review profile is part of the public footprint these systems read.

Directories and third-party mentions

In our Scottsdale runs, ChatGPT cited directory pages to justify almost every recommendation, and clinics present on the cited directories showed up more often. Not a guaranteed channel — OpenAI says placement is not guaranteed — but presence on the directories AI pulls from matters.

One layer that cuts across all four: consistency. Keep your clinic name, address, phone number, and core facts consistent across your site, Google Business Profile, directories, and other listings. We treat this as basic entity-consistency hygiene, not as a documented AI ranking factor.

The 100-Word Test

Most med spa pages mention the procedure, but price and a named provider are usually missing from the opening 100 words.

We call it the 100-Word AI Readiness Test: open a service page, read only the first 100 words, and check whether they answer the five questions a patient — or an AI — needs. Run across our research pool, most pages fail it. They open with a tagline and a headline, name the procedure, and stop before the facts a patient actually decides on. The two that go missing most are the two that matter most: what it costs and who performs it.

This is our check, not a documented rule from OpenAI or Google. What we can say is simple: a fact that is not in the first 100 words cannot be extracted from them.

Across all treatments the split is wider — 20% (22/108) vs 37% (25/68) — but that broader number may also reflect overall site quality, so we keep the eye on Botox.

We asked an AI search model with live web access to recommend med spas across 150 searches in 10 US metros, three times over, then grouped the clinics by their 100-Word score. The pattern was consistent: the more of those facts a page stated, the more often the clinic was named — cleanest for Botox, where the query matches the exact page we scored. It is an association, not proof, since reviews, ratings, and local ranking all move together with page quality. But it points the same way as everything else here — AI names the pages that hand it something concrete.

Why price is the payoff

Price is one of the most decision-useful facts an AI answer can carry, and when a patient asks, these systems answer. We watched it happen on Google, and we checked what those answers were built from.

Put it together and the mechanism is plain. Google answers the cost question every time and builds the answer from clinic pages, and overwhelmingly from the price-transparent minority. Most of the clinic pages it quotes contained a plain-text price for the queried treatment (76%); across med spas generally, only 45% do. A page that never states a price cannot itself supply that price to the answer; publishing it makes the fact available to quote.

Price is not the only gap, just the most common. “Call for pricing” is a deliberate conversion choice; the tradeoff is that neither a patient nor an AI can read a price that is not there. See our price-transparency study for the full scan.

Does schema help?

Schema can organize information for machines, and Google confirms its AI features use the same indexed web as Search. But it is not required and not a shortcut — Google’s own docs say no special structured data is needed. In our scan the machine-readable layer was often thinner than the visible page: everyone shipped the boilerplate and skipped the facts. Keep schema accurate, especially for provider and service, but do not treat it as the main event.

Test it in the AI itself, and track it

Open ChatGPT, or search the same patient-style query in Google and see whether an AI Overview appears, using prompts like these.

Then read the answer. Does it name your clinic? Are the facts about you accurate? Which competitors get named, and what do their pages state that yours does not?

Run the same prompts monthly in a fresh, logged-out session and log which clinics get named; that running list is your baseline AI visibility, so you can watch the trend. Keep the engine, prompt wording, city or location, and browser state consistent, and run each prompt more than once, because AI answers vary between runs even for the same query. One result is a signal rather than a verdict.

Methodology

Study A1 — the 100-Word Test

177 of 187 clinics had a scorable Botox or primary-injectables page. We read a strict first-100-words window and did not count facts stated lower on the page. Figures are dated observations from Sept 2026, not guarantees.

Study A2 — AI recommendation runs

We used an AI search model with live web access (gpt-4o-mini plus web) as a proxy for consumer AI search — not default ChatGPT — across 150 searches in 10 metros, three passes. Results are reported as an association, not causation. Sept 2026.

Study B — Google AI Overview price citations

We captured 50 cost AI Overviews. Source types were classified from citation titles and descriptions because the redirect URLs were encrypted, and the cited clinic pages (research 13.1) were resolved in a browser and crawled. All figures are dated September 2026 observations, not guarantees.
